Job summary

Qualifications

• Must possess excellent communication skills and the ability to interact with a diverse group of individuals.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Schedule visits and coordinate with field staff, patients and physicians.
• Manage staff schedules and handle scheduling reports.
• Assist with day-to-day office and staff management.
• Assist with weekly case conferences and referrals to Branch Director as needed.
• Maintain the patient hospitalization and coordination logs.
• Ensure supervisory visits are scheduled and reschedule missed visits.
