Job summary

Humana’s CenterWell Home Health in Florida seeks a Patient Services Coordinator to schedule visits and coordinate care among field staff, patients and physicians, ensuring continuity of care and smooth daily operations.

You will manage patient schedules, reassign visits, run scheduling reports, assist with weekly case conferences, and support office staff; 1 year of home health experience is required and a coding certification is a plus.

Qualifications

• Excellent communication with diverse groups, strong organizational skills, and ability to manage multiple tasks.
• At least 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification preferred.

Responsibilities

• Schedule visits and coordinate with field staff, patients and physicians to ensure continuity of care.
• Back up during lunch hours and busy times, handle field calls and assist with case conferences.
• Prepare and review weekly agent schedules and productivity reports.
• Run scheduling reports and ensure supervisory visits are scheduled.
• Assist with internal transfers of patients between branches.
