Job summary

Humana Inc. and CenterWell Home Health are seeking a Patient Services Coordinator to schedule visits and coordinate care among patients, field staff, and physicians. This role supports day-to-day office operations and staff management to maintain continuity of care.

The position requires strong communication, organization, and multitasking abilities with at least 1 year of home health experience. Occasional on-site responsibilities during busy periods are included.

Qualifications

• Must possess excellent communication skills and the ability to interact with a diverse group of individuals.
• Strong organizational skills with the ability to manage and prioritize multiple assignments.
• Must have at least 1 year of home health experience. Prior packet review / QI experience preferred. Coding certification is preferred.

Responsibilities

• Manages schedules for all patients and coordinates with field staff.
• Edits schedules when agents call in sick to ensure timely reassignments.
• Updates agent unavailability in the worker console and initiates infection control forms as needed.
• Acts as back-up during busy times, handles calls from field staff, and assists with weekly case conferences.
• Maintains the hospitalization log and coordinates with Medical Records and Case Manager as needed.
• Ensures supervisory visits are scheduled and assists with patient visits when required.
