Job summary

You will liaise between patients, providers, and referring physicians, help patients access services, coordinate referrals and follow-up care, and obtain insurance authorizations. Strong communication and teamwork are essential.

Responsibilities

• Provide administrative and patient support for assigned physicians and act as a key contact for patients throughout their care journey.
• Serve as liaison between patients, providers, and referring physicians to coordinate care.
• Assist patients with accessing services across the health system; coordinate referrals and follow-up care.
• Obtain, verify, and secure required insurance authorizations for services.
• Provide timely communication by phone and Epic/Care Connect; work with clinical and administrative teams to support care coordination and access to specialty services.
