HealthRIGHT 360 is seeking a dedicated Patient Safety Navigator in San Francisco, CA to welcome and guide patients through integrated behavioral health services and medical care. The role emphasizes maintaining a safe, respectful environment and coordinating patient flow across programs.

The PSN will de-escalate crises, escort clients to appointments, and serve as the first point of contact for new and returning clients, ensuring smooth enrollment and access to care in a mission-driven setting.

Qualifications

• HS Diploma or GED required.
• Community Health Worker or Mental Health related certificate strongly preferred.
• Familiarity with supportive counseling strategies and wellness concepts.
• Knowledge of San Francisco community behavioral and public health systems.
• 2–4 years of lived experience with community behavioral health or criminal justice systems.
• Ability to engage diverse, disadvantaged populations with barriers.
• Strong interpersonal relationships with colleagues and community.
• Understanding of social determinants of health and whole person care.

Responsabilités

• Welcomes and assists patients with navigating clinical resources within the building.
• Coordinates care and patient flow for timely services across HealthRIGHT360 programs.
• Explains available services, including rapid HIV/Hep C testing, TB screening, exams, addiction services, and primary medical/dental care.
• Escorts patients to appointments and provides excellent customer service.
• Assists patients with required enrollment paperwork for services.
• Coordinates transportation for patients in residential programs.
