Job summary

San Ysidro Health is seeking a Patient Navigator in San Diego to assist high-risk populations in accessing medical and social services, addressing psychosocial barriers, and promoting SYHealth programs. The role involves coordinating care, documenting interactions, and connecting patients to housing, transportation, and other essential resources.

Qualifications

• Associates Degree in social work or related field; experience may substitute.
• Minimum 1–2 years of professional patient navigation or care coordination in healthcare.
• Proficiency with Epic or similar EHR systems for scheduling, chart review, and data entry.
• Knowledge of health insurance processes, eligibility, and reimbursement.
• Experience conducting screenings for social determinants of health (SDOH) and connecting to resources.
• Ability to document interactions accurately and maintain compliance with regulations.

Responsibilities

• Screen for internal and external programs to educate, navigate, and connect patients with needed services.
• Provide services in-person at clinics and via telecommunication.
• Obtain consent and create patient profiles in the CIE system.
• Document all interactions and unmet needs in NextGen or equivalent system.
• Refer patients to social work, behavioral health, or other services as needed.
• Coordinate with interdisciplinary teams to ensure continuity of care.
• Travel to satellite facilities as required and maintain professional relationships with clients and partners.
