Custom Health Inc. is seeking a Patient Enrollment & Care Coordination Specialist for Finney's Institutional Pharmacy to manage onboarding, enrollment coordination, and guide patients through the pharmacy workflow from intake to completion.

The ideal candidate communicates clearly, multitasks effectively in a fast-paced environment, and delivers outstanding customer service while maintaining HIPAA-compliant records.

Qualifications

• Certified Pharmacy Technician (CPhT) required.
• Strong communication and customer service skills.
• Experience with enrollment workflows and HIPAA compliance.
• Ability to multitask in a fast-paced environment.

Responsabilités

• Answer inbound calls from patients, providers, and caregivers with enrollment questions.
• Make outbound calls to patients, physicians, insurers, and referral sources as needed.
• Collect, review, and enter patient information into internal systems.
• Coordinate and manage new patient enrollment workflows from intake through onboarding.
• Explain pharmacy processes and next steps clearly and professionally.
• Coordinate with internal departments to ensure timely referrals and prescriptions.
• Maintain confidential patient records in line with HIPAA guidelines.
• Follow up on missing documentation or required authorizations.
• Provide excellent customer service and build relationships with patients and partners.
• Adapt to changing priorities and assist with additional administrative tasks as needed.
• Other duties as assigned
