Custom Health is seeking a Patient Enrollment & Care Coordination Specialist for Finney's Institutional Pharmacy. The role focuses on onboarding new patients, coordinating enrollment activities, and guiding patients through the pharmacy workflow from intake to onboarding.

The ideal candidate is an excellent communicator who can multitask in a fast-paced environment while delivering exceptional customer service.

Qualifications

• Must have CPhT certification and Pharmacy Technician credentials.
• Strong communication and customer service skills are required.
• Experience coordinating enrollment workflows and onboarding is preferred.

Responsabilités

• Answer inbound calls from patients, providers, and caregivers.
• Make outbound calls to patients, physician offices, insurers and referrals.
• Accurately collect and enter patient data into internal systems.
• Coordinate new patient enrollment from intake to onboarding.
• Explain pharmacy processes clearly and professionally to patients.
