Job summary

OSF HealthCare is seeking a Patient Navigator to welcome new patients and connect them with the right provider, at the right location and time. You will guide patients through scheduling and referrals to ensure smooth access to primary and specialty care.

Training on site in Peoria with orientation; after that, option to work from home. As an ambassador for service excellence, you will collaborate across departments to deliver outstanding patient experiences and advocate for patient needs.

Qualifications

• Education: High School Diploma or GED.
• 2 years’ experience in a contact center, health, or public health setting.
• Excellent interpersonal and communication skills; proficient in MS Office.
• Strong customer service and organizational abilities.

Responsibilities

• Assist patients by connecting them with the right provider and location.
• Coordinate scheduling and referrals to ensure timely access to care.
