Job summary

PeaceHealth is seeking a Patient Navigator - Care Management for a Full Time, 1.00 FTE, Day position. The role focuses on improving access to healthcare and social services for patients with chronic or acute illness, coordinating care with a multidisciplinary team.

The position may be hybrid, allowing onsite and remote work, with duties including coordinating care plans, referrals, and patient education to ensure timely, appropriate services.

Qualifications

• Two years of post-secondary education or training preferred.
• Minimum of 2 years of experience in human services or healthcare preferred.
• Experience with healthcare systems and multicultural communities preferred.
• Bilingual/bicultural preferred.
• Basic Life Support certification required upon hire.
• Driver's license required upon hire.

Responsibilities

• Coordinates patient transportation and accompaniment to appointments, reducing barriers to care.
• Coaches patients on using a medical home and self-management of chronic conditions; follows up on care plans.
• Assists with development of patient education programs and tools and guides patients to resources.
• Tracks care across the continuum and ensures timely services by making referrals and guiding patients to care.
• Collaborates with Social Work, Behavioral Health, PCP, and Nursing to coordinate care.
• Engages with community social service agencies to improve patient access.
• Coordinates weekly care conferences with the primary care team for concise patient summaries.
• Screens patients for primary care eligibility, including private and safety net providers, and coverage.
