Sprinter Health is seeking a Care Navigator to perform high-volume outreach and coordinate care end-to-end, helping patients access services like mammograms and primary care while connecting them to social resources (SDoH).

The role requires strong communication, empathy, and the ability to work independently in a fast-paced environment. This temporary remote position runs through December 31, 2026, with a focus on seamless patient experiences across services.

Qualifications

• 4+ years in care navigation, care coordination, social work, call center ops, or patient outreach in healthcare/service environments.
• Able to work independently in a high-volume, fast-paced setting.
• Adaptable to evolving workflows, tools and priorities.
• Strong critical thinking and problem-solving skills.
• High emotional intelligence and empathy in patient interactions.
• Experience with SDoH or community resource navigation preferred.
• Excellent written and verbal communication skills.
• Experience managing multiple channels (phone, email, chat).
• Startup or high-growth experience is a plus.

Responsibilities

• Conduct high-volume outbound calls to engage patients and schedule healthcare services.
• Coordinate care across services (mammograms, bone density, primary care) and ensure scheduling, follow-up, and completion.
• Identify and address SDoH barriers by connecting patients to community resources.
• Conduct patient assessments to understand clinical, social, and logistical barriers.
• Educate patients on services and resources and how to access them.
• Build trust with patients and caregivers through clear, empathetic communication.
• Proactively follow up to ensure completion and close gaps in care.
• Manage multiple workflows with accuracy and attention to detail.
• Maintain detailed documentation of patient interactions per HIPAA standards.
• Collaborate with clinical, operations, and support teams for seamless experiences.
• Escalate issues promptly when risks or blockers arise.
• Support additional projects as needed.
