Qualifications

Responsibilities

• Serve as the first point of contact for new patients entering Waterford Smilow Care Center.
• Process new patient referrals, intake and entry of demographics and insurance data.
• Compile patient records for initial consult and coordinate with Nurse Coordinator.
• Schedule visits and arrange pre-visit testing.
• Register accounts and ensure insurance referrals are in Epic.
• Send new patient packets by email or mail.
• Orient patients and families to facilities and visit process.
• Identify additional access to care and coordinate referrals.
• Notify referring providers of appointment details.
• Field inquiries related to new patients and scheduling.
• Place live reminder calls for new consults.
• Meet performance expectations in customer service and teamwork.
• Perform other duties to ensure smooth operation.
