Job summary

Maxim Healthcare is seeking a compassionate Intake Coordinator to join our team. This role focuses on ensuring timely and accurate patient care by coordinating services and managing patient intake.

You will work with the Client Coordinator and staff to maintain high standards of customer service. Key responsibilities include processing referrals, documenting patient needs, communicating available services, and scheduling personnel.

Qualifications

• High School Diploma or equivalent required.
• LPN licensure and/or college degree preferred.
• Minimum of 1 year of relevant experience preferred.
• Ability to travel up to 25%.

Responsibilities

• Capture and communicate patient referral information to management teams.
• Process and document incoming patient needs, service requests, and changes.
• Inform patients accurately about available services.
• Collaborate with operations to schedule appropriate personnel promptly.
• Assist with new case openings in patient homes.
• Record and report patient and employee concerns to appropriate parties.
• Enter patient data into the system of record per company policies.
• Support office staff with routine administrative duties.
• Perform other duties as assigned.
