Job summary

Maxim Healthcare is seeking a compassionate Intake Coordinator to ensure timely, accurate patient care. You will manage intake, coordinate services and maintain high customer service standards, working with the Client Coordinator and staff.

Qualifications include a HS diploma or equivalent; LPN licensure or degree preferred, and at least 1 year of relevant experience. Travel up to 25% is required; a strong telephone manner and MS Office skills are essential.

Qualifications

• High School Diploma or equivalent required; additional education preferred.
• LPN licensure or college degree is preferred but not required.
• Strong organizational and communication abilities are essential.

Responsibilities

• Capture and communicate patient referral information to management teams.
• Process and document incoming patient needs, service requests, and changes.
• Inform patients accurately about available services.
• Collaborate with operations to schedule appropriate personnel promptly.
• Assist with new case openings in patient homes.
• Record and report patient and employee concerns to appropriate parties.
• Enter patient data into the system of record per company policies.
• Support office staff with routine administrative duties.
• Perform other duties as assigned.
