Job summary

Maxim Healthcare is seeking a compassionate Intake Coordinator to join our team in California, coordinating patient intake, services and ensuring top-quality customer service.

The role collaborates with the Client Coordinator, processes referrals, documents patient needs, and supports office staff with routine duties while delivering excellent bilingual communication in English and Spanish.

Qualifications

• High School Diploma or equivalent required; further education preferred.
• Minimum 1 year of relevant intake/coordination experience preferred.
• Ability to travel up to 25% is noted.
• Fluent in English and Spanish is required.

Responsibilities

• Capture and communicate patient referral information to management teams
• Process and document incoming patient needs, service requests, and changes
• Inform patients accurately about available services
• Collaborate with operations to schedule appropriate personnel promptly
• Assist with new case openings in patient homes
• Record and report patient and employee concerns to appropriate parties
• Enter patient data into the system of record per company policies
• Support office staff with routine administrative duties
• Perform other duties as assigned
