Jefferson Health is seeking a Patient Navigator to serve as a liaison for patients, coordinating with physicians and staff to determine services, schedule appointments across departments, and follow up to assure compliance. This role supports delivering the Jefferson Experience through prompt, courteous care for all patients and visitors.

The navigator will interact with co-workers, schedule future visits, perform screenings as needed, and ensure HIPAA compliance while maintaining relationships

Qualifikationen

• 3 years of experience in a medical office.
• Must be able to communicate courteously with patients/visitors.
• Willingness to provide and support a positive Jefferson Experience for all patients/visitors.
• Leadership skills to include proactivity and problem solving.
• Ability to establish and develop positive and collaborative relationships.

Aufgaben

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Schedules patients for all visits undergoing future care
• Administers screenings and intake assessments as needed
• Monitor treatment plan adherence
• Triage patient phone calls
• Coordinate inpatient care as needed
• Develop, maintain, update community resource list
• Greets and interacts with patients/visitors in a professional manner, on phone or in person.
• Adheres to HIPAA privacy and security requirements.
• Ensures compliance with all applicable federal, state, and local regulatory standards.
• Other duties as requested or assigned.
