Job summary

Jefferson Health is seeking a Patient Navigator to act as a liaison for Jefferson patients, coordinating services with physicians and staff and ensuring timely appointment scheduling across departments. The role emphasizes compassionate patient interactions and adherence to privacy standards.

Responsibilities include coordinating inpatient care as needed, following up to ensure treatment adherence, and maintaining community resource lists.

Qualifications

• 3 years of experience in a medical office.
• Must communicate courteously with patients and/or visitors.
• Leadership skills including proactivity and problem solving.
• Bachelor’s degree in psychology, health administration or related field; Epic experience preferred.

Responsibilities

• Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
• Schedules patients for all visits undergoing future care.
• Administers screenings and intake assessments as needed.
• Monitor treatment plan adherence.
• Triage patient phone calls.
• Coordinate inpatient care as needed.
• Develop, maintain, update community resource list.
• Greets and interacts with patients/visitors professionally, on phone or in person.
• Adheres to HIPAA privacy and security requirements.
• Ensures compliance with federal, state, and local regulatory standards.
• Other duties as requested or assigned.
