Job summary

Qualifications

LPN license required.
Minimum 2 years nursing experience in Emergency Department/Trauma.
Valid driver’s license and BLS certification.
Strong written, verbal, and interpersonal communication skills.
Proficiency reviewing medical records and EPIC.
Ability to promote teamwork and multidisciplinary collaboration.

Responsibilities

Patient Experience & Advocacy: ensure compassionate, seamless patient experiences and promote informed engagement.
Care Coordination: coordinate referrals and transitions to ensure timely care.
Clinical Support & Navigation: triage new patient referrals and guide patients through the system.
Education & Communication: provide tailored education on diagnoses, treatments, symptoms, and post‑treatment needs.
Barrier Reduction & Transition Support: identify obstacles and support safe care transitions.
Assessment & Documentation: assess needs and document in EPIC with accuracy.
Leadership & Quality Improvement: contribute to staff education and process improvements.
