Job summary

Signature Health is seeking a Patient Navigator to support new and returning patients, coordinating linkage to medical and behavioral health services and acting as a primary point of contact. You will document encounters in the EHR and collaborate with care teams to ensure seamless, integrated care for vulnerable populations, while maintaining productivity and privacy obligations.

Applicants should have relevant behavioral health experience and preferably an Associate's degree.

Qualifications

• Two years of experience in a behavioral health environment or equivalent education
• Epic experience a plus
• Verbal and written fluency in Spanish and English a plus

Responsibilities

• Provide an exceptional experience for patients by phone and in person
• Coordinate and link patients to needed medical and behavioral health services
• Document patient encounters in the EHR in a timely manner with billable interventions
• Work with care teams to ensure quality integrated care
• Help maintain patient retention and productivity goals while protecting privacy
