Job summary

Upperline Health is hiring a Practice Success Coordinator to support our value-based care program, Upperline Plus. This onsite, patient-facing role involves educating patients, enrolling them in the program, and collaborating with clinic staff to improve enrollment workflows.

You'll work at partner practices, explain program benefits during check-in/check-out, help patients schedule visits, and build rapport with teams.

Qualifications

• High school diploma or equivalent.
• 2+ years of customer service experience.
• Comfort with iPads and Microsoft Office.
• Strong communication skills and HIPAA awareness.

Responsibilities

• Educate and enroll patients in the program.
• Help patients see the value of engaging in the program and guide them through scheduling visits.
• Build rapport with patients and clinic teams.
• Partner with clinic staff to troubleshoot issues and refine enrollment workflows.
• Represent Upperline's culture and values at partner sites.
