Job summary

Upperline Health is hiring a Practice Success Coordinator to support our value-based care program, Upperline Plus. This onsite, patient-facing role educates patients and guides enrollment using an iPad.

You'll collaborate with clinic staff to hit enrollment and engagement goals, improve workflows, and represent Upperline's culture at partner sites. No sales experience required; strong communication is key.

Qualifications

• 2+ years of customer service experience.
• Comfort with iPads and Microsoft Office.
• Strong communication skills and HIPAA awareness.

Responsibilities

• Educate and enroll patients in the program.
• Help patients see the value and schedule visits.
• Build rapport with patients and clinic teams.
• Partner with clinic staff to refine enrollment workflows.
• Represent Upperline's culture and values at partner sites.
