Job summary

Valeris is seeking a caregiver coordination professional to support patients in navigating benefits, coverage, and adherence programs. Qualifications

• Bachelor's degree strongly preferred or equivalent experience.
• Customer service and healthcare industry experience strongly preferred.
• Ability to work an 8.5-hour shift between 8 am to 9 pm, Monday to Friday.
• Bi-lingual in English and Spanish or Chinese/Mandarin is a plus.
• Commitment to the mission of care coordination.
• Good communication and interpersonal skills; speak concisely to patients.
• Professional telephone etiquette.
• Self-awareness of emotions and impact on others.

Responsibilities

• Build and maintain trusted relationships with patients with proactive communication.
• Ensure all interactions comply with healthcare laws and regulations.
• Explain coverage options and pharmacy benefit products to patients.
• Conduct program welcome calls and engage patients effectively.
• Utilize motivational interviewing to involve patients in goals.
• Document patient enrollment and maintain accurate records.
• Provide education about products, insurance coverage, and costs.
• Coordinate shipments via patient assistance programs where needed.
• Support adherence services through nurse follow-up when applicable.
• Identify and suggest peer support resources for patients.
