Job summary

Apicha Community Health Center is seeking a Patient Engagement Specialist in Jackson Heights, NY to manage new patient enrollment and re-engagement in care. The role requires coordinating internal and external referrals and using CRM tools to boost patient engagement and access to services.

The PES will take inbound referrals, schedule intakes, make follow-up calls, and address care gaps. Strong communication, time management, and bilingual abilities are valued in a culturally competent team.

Qualifications

• High school diploma/GED required; college education preferred.
• Bilingual (Spanish or Mandarin) preferred.
• Experience in customer service or sales is preferred.

Responsibilities

• Handle inbound referrals from patients or health care organizations; describe Apicha services and schedule intakes.
• Make follow-up outbound calls to discuss services and schedule appointments.
• Conduct outreach to address care gaps and improve engagement.
• Utilize CRM tools to log calls and scheduling with high-quality service.
