Job summary

Apicha Community Health Center in New York, NY is seeking a full-time Patient Engagement Specialist for our Manhattan location. The role focuses on improving patient engagement through referrals, CRM tools, and clear communication.

You will take inbound referrals, schedule intakes, and follow up with patients to discuss services. Proficiency with CRM systems and empathy are essential, as is the ability to work 9:00 a.m. - 7:30 p.m. EST.

Qualifications

• High school diploma or GED required; college education preferred.
• Bilingual in Spanish or Mandarin preferred.
• Proficient with computer systems and CRM tools.
• Experience in customer service or sales preferred.
• Strong written and verbal communication skills.

Responsibilities

• Take inbound referrals from patients or health care organizations, describe Apicha services, schedule new patient intakes and refer to other services as needed.
• Make follow-up outbound calls to discuss Apicha's services and schedule appointments.
• Utilize phone, scheduling, and CRM tools to manage call results data and scheduling.
• Leverage established conversation guides to engage, connect, and educate callers.
• Document patient call interactions and schedule intakes,Maintain quality customer service.
