Queen Emma Clinic is seeking a Patient Community Navigator to support social determinants of health needs for clinic patients. The role involves building trusting relationships with patients and families, coordinating resources, and performing home visits as needed.

Responsibilities include off-site work in the community and adherence to clinic policies. Must have a valid driver’s license and be able to travel to patient residences as required.

Formación

• High school diploma or equivalent required.
• Current BLS/HCP certification required.
• Certified Medical Assistant, Community Health Worker or Patient Navigator preferred.
• Valid U.S. driver’s license and clean driving record required.
• Must comply with fleet safety policy.

Responsabilidades

• Supports community health by building trust with patients, families, and providers.
• Coordinates resources and removes barriers related to social determinants of health.
• May perform activities off-site, including home visits to patients.
• Works in alignment with Queen Emma Clinic and The Queen’s Medical Center policies.

Educación

High School diploma or equivalent

BLS/HCP certification

Medical Assistant/CHW/Patient Navigator certification preferred

Driver's license current and clean driving record

Fleet Safety Policy compliance
