CommonSpirit Health is seeking a Community Health Worker to bridge underserved communities with essential resources. You will conduct home visits, assess needs, and connect clients to medical, behavioral health, and social services.

Qualifications

• High School Diploma or GED required.
• Minimum of 1 year of experience in social services or care coordination.
• Driver's license valid in California.
• Community health worker training or certification preferred.

Responsabilités

• Advocates for individual and community health needs.
• Builds capacity through outreach, education, counseling, and advocacy.
• Conducts standardized client intake and risk assessments.
• Develops and updates plan of care based on client goals.
• Participates in assessments via in-person, home, office, or telehealth visits.
• Collaborates with health providers and housing/legal advocates to coordinate services.
• Identifies and assists enrollment in eligible health and human services programs.
