Job summary

The Queen's Health Systems seeks a dedicated community health professional to foster trusted relationships with patients, families, and providers, promoting health, recovery, resiliency, and wellness. The role includes off-site activities and home visits to support community care.

Qualifications include a high school diploma, a valid U.S. driver’s license with a clean driving record, and current BLS certification. Certification in Medical Assistant/CHW/Patient Navigation is preferred.

Qualifications

• High School diploma or equivalent.
• Valid U.S. driver’s license is required.
• Driver's abstract or good driving record.
• Medical Assistant, Community Health Worker or Patient Navigation certification preferred.
• Current BLS certification.
• Two years of healthcare/social services experience; or one year with CHW certificate.
• Experience with behavioral health and/or substance use preferred.
• Experience with Native Hawaiian and underserved populations preferred.

Responsibilities

• Supports community health by establishing trusting relationships with patients, families, and providers to promote health and wellness.
• Performs activities off-site, in the community, including home visits when required.
• Adheres to policies and procedures aligned with the department and QUMG goals.
