Equitas Health is seeking a qualified Care Navigator to support patients through social determinants of health and navigate medical and community resources. The role requires traveling, driving, and thorough documentation to ensure timely care and benefit access.

You will collaborate with providers and the care team to develop care plans, track outcomes, and maintain confidentiality while contributing to training and committee activities.

Qualifications

• Must have sensitivity to cultural differences and competence in working with persons of color and LGBTQ communities.
• Medical and community-based care navigation training experience desired.
• Understanding of value-based care delivery models preferred.
• Experience working in a multidisciplinary team required.
• proficiency in Microsoft Office applications.
• Reliable transportation, driver’s license and proof of auto insurance required.

Responsabilités

• Promotes and reinforces patient centered medical home concepts with patients, families, and the Equitas Health care team.
• Assist patients in accessing Equitas Health resources and navigating external healthcare and community resources.
• Collaborates with providers, client services, and care team to develop care plans and strategies.
• Collects data through patient tracking to meet patient needs and outcomes.
• Provide assistance with documentation related to patient care, including Ryan White benefits and referrals.
• Maintain confidentiality of patients per HIPAA and policies.
• Attend trainings and participate in committees to support professional development.
