Family Health Centers of San Diego is seeking a Patient Health Navigator to support linkage and maintenance in care, including referrals, for assigned programs. You will schedule home visits, assist with prior authorization, and coordinate care through a cooperative, multidisciplinary team.

You will engage with patients and families, gather scheduling information, and ensure accurate documentation in the EMR. A driver’s license and travel within San Diego County may be required.

Qualifications

• High School diploma or GED required.
• 2 years of experience in health education or the medical/health field preferred.
• Bilingual English/Spanish may be required depending on assignment.
• Active Class C California driver’s license with proof of liability insurance may be required.

Responsabilités

• Coordinate and schedule daily visits for the Tiger team responsible for home based medical services.
• Create Patient home visit schedules that are effective and maximize visits.
• Manage multiple patient scheduling related to cancellations and events to maximize productivity.
• Assist Patients in assessing barriers to care and plan steps to overcome them.
• Gather information related to scheduling home visit appointments via phone, in person, or email.
• Provide excellent customer service for Patients and Tiger team.
• Complete required documentation accurately and timely per department standards.
• Liaise between patients, Tiger team, and service providers to improve care flow.
