Job summary

The Community Health & Wellness Center is seeking a motivated Community Health Worker to help patients and families access community and social services, promote healthy behaviors, and advocate within the care team.

You will conduct outreach, home visits, health screenings, assist with applications, enroll individuals in insurance programs, and coordinate referrals, all while maintaining patient confidentiality and documenting in the EMR.

Qualifications

• High School Diploma or equivalent.
• Completion of Community Health Worker program certification preferred.
• 3 years of health and/or social service experience.
• Knowledge of the community served.
• Information Technology skills such as laptops, smartphones and Microsoft Office.

Responsibilities

• Create connections between vulnerable populations and healthcare providers to ensure patients have comprehensive and coordinated care.
• Work cooperatively with other clinical staff assigned to the same patient.
• Manage assigned caseload of patients.
• Record patient care management information in the EMR no later than 24 hours after contact.
• Attend regular staff meetings, trainings and other meetings as requested.
• Assist with completing applications, registration or other forms.
• Register new patients, schedule appointments and upload insurance information while doing outreach on Mobile Medical Unit.
• Determine eligibility and enroll individuals in health insurance and/or financial assistance programs.
• Foster collaboration with community service agencies to address social and healthcare needs.
