Job summary

Circle the City in Phoenix, AZ is seeking a Patient Navigator to collaborate with the Integrated Health Care team. You will address social determinants of health, help with appointment scheduling, transportation, housing, and access to services, and advocate for patients across community partners.

Qualifications

• High school diploma or GED required.
• Must be 21 years of age.
• Arizona driver’s license required.
• Arizona Department of Safety Clearance Card required.
• Basic Life Support (BLS) Certification.
• Spanish speaking preferred.
• Excellent oral and written communication skills.

Responsibilities

• Refer and connect patient to unmet social determinants of health.
• Assess eligibility for DES benefits and coordinate enrollment.
• Assist patients in understanding care plans and document progress.
• Provide education and support to patients with chronic conditions.
• Build relationships with patients and community partners.
• Document activities in EMR systems.
• Participate in team meetings and quality improvement activities.
• Ensure HIPAA compliance during all interactions.
