Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.
• Must possess a high school diploma or equivalent.
• Must be a licensed practical or vocational nurse in the state of practice or have recent home health/hospice experience and staffing knowledge.

Responsibilities

• Schedule patients to branch field clinicians and coordinate with staff, patients, physicians, referral sources, and caregivers to ensure continuity of care.
• Manage the on-call notebook and hospitalization logs to improve communication among stakeholders.
