Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to align patient appointments with branch field clinicians and ensure seamless care coordination. You will serve as a liaison among field staff, patients, physicians and referral sources to uphold high-quality service.

Qualifications

• Must have a high school diploma or equivalent.
• Must either be a licensed practical or vocational nurse in the state of practice with at least 1 year of clinical experience, or have at least one year of home health, hospice, or pediatric experience and understanding of staffing and scheduling requirements.

Responsibilities

• Schedule patients to branch field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
