Job summary

Qualifications

• High school diploma or equivalent required.
• LPN/LVN license in state with 1 year clinical experience or 1 year home health/hospice/pediatrics experience in last 24 months with understanding of staffing and scheduling.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Schedule patients to branch field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to ensure care coordination.
• Maintain on-call notebook and hospitalization logs to improve stakeholder communication.
