Job summary

SouthEast Alaska Regional Health Consortium (SEARHC) is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, collaborating with the care team to ensure treatment plans are followed and patient needs are met.

The role focuses on coordinating referrals, communicating plan of care, and supporting a patient-centered, efficient clinic flow in Alaska’s Southeast region. Strong organization, teamwork, and EHR proficiency are essential.

Qualifications

• 2 years of healthcare, behavioral health, dental, or relevant administrative experience required.
• Internal candidates with at least 2 years in front/back-office roles may be considered.
• Basic Life Support preferred.

• Assists the care team in coordinating safe, timely, effective, efficient, equitable, patient-centered care.
