Job summary

SEARHC is looking for a Primary Care Coordinator to coordinate comprehensive patient care, liaise with the care team, manage referrals, and support plan-of-care communications. The role emphasizes patient-centered service and timely follow-up within the Southeast Alaska health system.

Qualified candidates will have an associate degree or equivalent experience, strong communication skills, and experience with electronic health records; a Bachelor’s degree is acceptable in place of experience.

Qualifications

• Associate’s degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field; OR 2 years of relevant professional experience.
• A Bachelor’s degree may be exchanged for required experience; Basic Life Support preferred.

Responsibilities

• Coordinate safe, timely, patient-centered care with the care team.
• Manage internal and external referrals and communication of Plan of Care (POC).
• Perform patient outreach by phone or text to schedule with the care team.
• Maintain data on case management and care outcomes per SEARHC standards.
