Job summary

SEARHC is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, ensuring treatment plans are followed and patient needs are met. The role collaborates with care teams and community resources to maintain a seamless Plan of Care.

The position emphasizes outreach, referrals, and clear communication with patients and caregivers, within a supportive nonprofit health consortium in Southeast Alaska.

Qualifications

• Associate’s degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field.
• Or 2 years of relevant professional experience may be exchanged for a degree or course work.
• A Bachelor’s degree in a relevant field may be exchanged for all required experience.

Responsibilities

• Assists the care team in coordinating safe, timely, effective, patient-centered care.
• Manages internal and external referrals and communicates Plan of Care to patients.
• Performs patient outreach by phone or text to schedule appointments with the care team.
• Maintains data on case management and care coordination.
• Ensures safety, infection control, and compliance with competencies.
