• Use patient registries and payer reports to identify outreach needs.
• Enter data into payer portals and EHR as documented.
• Apply HEDIS quality measures in daily operations.
• Document patient interactions accurately in EHR.
• Attend quality meetings and payer trainings.
• Maintain organized records supporting quality metrics.
