Qualifications

• LPN or higher certification required to apply.
• Excellent communication, organization, and problem-solving skills.

Responsibilities

• Handles inbound/outbound calls and emails from customers and patients.
• Answers phone courteously, records caller data and nature of business.
• Documents in the EHR and forwards messages to care teams accurately.
• Obtains prior authorizations for meds/tests/referrals as needed.
• Maintains confidentiality and follows HIPAA guidelines at all times.
• Supports care coordination and case management activities as directed.
• Responds to crisis calls and routes to appropriate care team members.
• Participates in training and quality improvement activities.
