Qualifications

Responsibilities

• Coordinate and schedule patients for Annual Wellness Exams and close clinical care gaps.
• Support participation in value-based programs to improve quality outcomes.
• Utilize patient registries and payer lists to track patients needing outreach or gap closures.
• Transcribe and submit patient data from the EHR into payer portals to ensure quality measures are captured.
• Document patient interactions accurately in the EHR and maintain records.
• Communicate effectively with patients, providers, and payer representatives to ensure smooth coordination.
