Job summary

Qualifications

• Successful completion of a Community Health Worker formal training program (preferred).
• Bachelor’s degree in Health Education, Health Promotion, Psychology, or Social Work, or equivalent combination of training/experience.
• Strong written and verbal communication skills are required.
• Ability to work with minimal supervision and manage multiple priorities.
• Valid Alabama Driver’s License is required.
• Familiarity with social service agencies and Medicare/Medicaid regulations is preferred.

Responsibilities

• Provide outreach in health fairs, clinic outreach, and targeted outreach lists.
• Deliver culturally appropriate health education and information, often using EHR tools.
• Assist clients with resource navigation, appointments, and program applications.
• Administer evidence-based screening tools to identify mental health or substance abuse needs.
• Assist patients with enrollment, referrals, and transportation; coordinate with bilingual providers or translators.
• Support care team by tracking and coordinating patient services across settings.
• Collaborate with clinical staff to contact patients via protocol-driven outreach.
• Provide psychoeducation on mental health symptoms, treatments, and coping strategies.
