Job summary

MedStar Health operates MedStar Washington Hospital Center in Washington, DC, seeking an organized Care Navigator to coordinate patient scheduling and care navigation. This role interfaces with patients, physicians, and care teams, arranging follow-up visits and referrals, and ensuring timely, accurate documentation.

You will work in a hybrid model after training, Monday–Friday 8am–4:30pm, providing exceptional service, maintaining patient relations, and supporting leadership with process

Qualifications

• High school diploma or GED is required.
• 2 years of experience in a medical office coordinating patient care.
• Strong communication and customer service skills.
• Familiarity with Excel and basic computer skills.

Responsibilities

• Schedules hospital and Urgent Care follow-up visits, specialists, and procedures.
• Coordinates follow-up care with clinical teams across MedStar entities.
• Acts as a Patient Relations Representative promoting patient experience.
• Maintains documentation in the patient chart and trackers.
• Provides friendly, professional customer service and resolves patient concerns.
