Responsibilities

• Coordinate the plan of care across the continuum with the care team.
• Assist with scheduling ancillary testing and services and presenting necessary information.
• Act as a patient advocate to identify and eliminate barriers to care.
• Support quality improvement initiatives and monitor patient outcomes.
