Job summary

MedStar Health is seeking a Clinical Navigator at MedStar Washington Hospital Center. The ideal candidate will be a Nurse, Physical Therapist, Occupational Therapist, or Social Worker with leadership skills to plan, direct, and improve patient care coordination throughout the acute and post-acute continuum.

The Clinical Navigator will collaborate with patients, families, and healthcare teams to determine the most appropriate post-acute environment, supporting timely and medically appropriate

Qualifications

• Nurse,PT/OT, or Social Worker with leadership and initiative.
• Experience coordinating pre-/post-acute care.
• Ability to educate patients/families and coordinate care plans.
• Knowledge of discharge planning and care transitions.

Responsibilities

• Collaborates with extended team to develop pre-operative and post-acute care plans.
• Coordinates pre-surgical prep and post-acute care with MedStar associates and clinicians.
• Educates patients and families on care programs and transition processes.
• Maintains relationships with physicians and referral sources to support teamwork.
• Monitors daily census, rounds, and documents patient care and workflow improvements.
