Job summary

Capital Medical Group, part of Independence Physician Management within Universal Health Services, seeks an experienced Clinical Nurse Navigator to support ambulatory care across the Washington, DC area. The role combines direct patient nursing with care coordination and triage.

You will work onsite with physicians and the clinical team, manage patient communications, education, and follow-up planning to ensure timely, high-quality care.

Qualifications

• Associate's degree or higher in Nursing, or an equivalent combination of education, training, and experience.
• Current and valid District of Columbia license as a Registered Nurse (RN), or evidence of application for D.C. licensure as permitted by organizational policy.
• Maintain all licenses and certifications required for assigned nursing responsibilities.
• Minimum of two (2) years of experience as a Registered Nurse.
• Ambulatory care, primary care, internal medicine, family medicine, care coordination, or other relevant clinical nursing experience preferred.
• Direct patient-care experience strongly preferred.
• Experience providing patient education, telephone or electronic clinical triage, medication management support, and care coordination preferred.
• Experience working collaboratively with physicians and advanced practice providers in an ambulatory clinical environment preferred.

Responsibilities

• Provide direct, patient-facing nursing care within the ambulatory clinical setting in accordance with established nursing standards, provider plans, and RN scope of practice.
• Maintain an active presence within assigned clinics and work with physicians, advanced practice providers, and medical assistants to support safe patient care.
• Perform nursing assessments and collect pertinent clinical information for clinical decision-making.
• Identify changes in patient condition and escalate when needed.
• Support medications management and provide education to patients.
• Administer medications, injections, or other nursing interventions as ordered.
• Provide patient education and counseling during or after ambulatory visits.
• Document clinical information in the electronic medical record (EMR).
• Ensure continuity between in-person visits and follow-up care.
• Maintain infection prevention and patient safety standards.
