Qualifications

• Associate's degree in Nursing required.
• Bachelor's degree in Nursing preferred.
• For candidates with an Associate degree, enrollment in an accredited BSN program within 1 year and BSN completion within 5 years is required.
• 3-4 years of progressively more responsible clinical nursing experience in a specialty area.
• RN license in DC required (State Licensure/Compact).
• CPR certification for healthcare providers within 90 days (AHA/Red Cross).
• Professional certification in a clinical area may be required within 1 year of hire (OCN/CCRN, etc).
• Ability to work in an interdisciplinary team and communicate effectively.

Responsibilities

• Assessment: obtain a baseline of information for individualized care plans.
• Plans: develop and manage the plan of care with an interdisciplinary team.
• Coordinate scheduling of ancillary testing and services within the care continuum.
• Assist at‑risk patients and families with risk assessment, screening and education.
• Coordinate continuing care including home care, palliative and hospice services as needed.
• Provide initial and ongoing education for disease processes and treatment plans.
• Serve as a resource for community education and outreach development.
• Act as liaison to coordinate care among patients, families, team, and facilities.
• Advocate for patients to remove barriers to care and ensure timely referrals.
• Monitor transition of referrals to specialty services and ensure timeliness.
• Connect patients with resources and support services across MedStar Washington Hospital Center and community partners.
• Support National Patient Safety Goals and nursing quality indicators with the team.
• Deliver direct patient care as required.
• Evaluate patient progress, goal attainment, and satisfaction to adjust plans.
