Job summary

Hanger Clinic is seeking a Community Care Coordinator in Indianapolis, IN to deliver proactive patient support and exceptional service. The role focuses on coordinating care before and after prosthetic/orthotic fittings, working with surgeons, care teams and referral partners to ensure a smooth patient experience.

You will assist with scheduling, patient education, and clinical documentation, while maintaining strong relationships with patients and providers.

Qualifications

• Minimum 2 years of experience in a patient care environment.
• BA/BS in Health related field or equivalent work experience.
• Ability to be credentialed in major hospital systems.
• Experience delivering high level customer service in fast-paced settings with discretion.

Responsibilities

• Contact new patients before/after initial consult or AmpuShield fitting and enter into patient management system.
• Support amputee patients with information, resources and counseling through pre/post-surgery.
• Coordinate visits and maintain referral relationships for the clinic.
• Assist with clinical documentation for insurance coverage and approvals.
• Maintain patient engagement with follow-up communications and appointments.
• Provide updates to surgeons and care teams with wound status and care plans.
• Complete hospital or facility visits as needed by the care team.
• Identify referrals to ensure continuity of care for patients and families.
• Act as liaison between patients and clinic areas to coordinate pre/post-operative care.
• Utilize CCC tools to communicate with patients and health professionals.
• Help patients with upcoming visits and fittings, potentially ABC Certified Orthotic Fitter.
