Job summary

Hanger, Inc. in Indianapolis is seeking a Community Care Coordinator (CCC) to deliver superior patient support and coordinate care for amputees and limb-difference patients within Hanger Clinic.

You will work under ACM/CM, build relationships with providers, manage pre/post-surgery care, and ensure exceptional patient experiences through proactive communication and thorough care coordination.

Qualifications

• Minimum 2 years in a patient care environment.
• BA/BS in Health related field or equivalent work experience.
• Ability to credential in major hospital systems.

Responsibilities

• Coordinate and manage patient care communications and referrals.
• Support patients with pre- and post-surgery processes and insurance coordination.
• Build relationships with referral partners and clinicians to improve patient care.
• Maintain patient engagement through follow-up communications and appointments.
