Job summary

Hanger Clinic is seeking a Community Care Coordinator in Durham, NC to provide proactive patient support, advocacy, and education for limb loss and orthotic/prosthetic needs. You will coordinate care, manage referrals, and maintain patient relationships, working closely with physicians, care teams, and external partners.

The role emphasizes exceptional patient experience, coordination of pre/post-surgical processes, and effective communication across care settings.

Qualifications

• Minimum 2 years of experience in a patient care environment.
• BA/BS in Health related field or equivalent work experience.
• Ability to be credentialed in major hospital systems.
• Must have, or be eligible to obtain, a valid driver’s license and driving record within policy.
• Experience in providing a high level of customer service in a fast paced environment, including handling confidential data with discretion.
• SharePoint Post-Acute CRM versed.
• Understanding of medical reimbursement and terminology.
• ABC Certified Orthotic Fitter or related credential is a plus.

Responsibilities

• Contact new patients before or after initial consult and enter them into the patient management system.
• Support patients with amputations during pre/post-surgery with information and guidance.
• Coordinate AMPOWER visits and build referral relationships for Hanger Clinic.
• Assist with obtaining clinical documentation for insurance coverage.
• Maintain follow-up communications to ensure an excellent patient experience and continuity of care.
• Coordinate pre-operative and post-operative planning and care with care teams and external providers.
• Use CCC tools to communicate with patients and health professionals.
