This full-time role focuses on outbound/inbound patient follow-ups, coordinating care, and keeping internal tickets up to date. You will report to the Care Coordination Manager and work with multiple departments to ensure timely responses, quality service, and patient satisfaction.

Qualifications

• High School diploma or equivalent.
• Strong verbal and written communication skills.
• Ability to maintain confidentiality.
• Excellent customer service and organizational skills.
• Ability to meet deadlines and work independently.

Responsibilities

• Manage outbound and inbound calls for customer follow up and success plans.
• Follow up on patient requests and internal tickets with care coordinators.
• Maintain department metrics and ensure timely resolution of tickets.
• Mentor new employees joining the department.
